The Role of Yoga in Cognitive Health in Older Adults: Opportunities for Inclusive Integrative Care

Yoga is a spiritual discipline originating in ancient India, rooted in the connection between the mind, body, and spirit. This practice includes physical postures (āsanas), meditation (dhyana), and breathwork (pranayama), providing a feasible and safe modality for mental and physical focus in aging communities. In exploring person-centred, age-friendly care, yoga offers an opportunity to integrate nonpharmacological strategies into positive cognitive interventions for global aging populations.

Aging is a series of dynamic lifestyle processes that produce diverse physiological changes. While aging is often characterized by mental or physical decline, new research highlights strategies to enhance healthy aging and mitigate loss of cognitive functions. These alternative outcomes can be found outside of Western pharmacology through integrated, holistic, and accessible care options such as yoga.

Yoga’s origins remain a highly contested subject among scholars. Some researchers have proposed that proto-yogic practices can be traced to the Indus Valley Civilization (c. 2500 BCE), referencing artifacts such as the Pashupati Seal, which displays a figure demonstrating possible meditative poses and āsanas. Other scholars favor a gradual development model of yoga’s origins rather than a single point of invention. This perspective suggests that yoga evolved over centuries of knowledge exchange between Vedic ritual culture, the philosophical teachings of the Upanishads, and the śramana traditions associated with early Hindu, Buddhist, and Jain practices. Despite scholarly debate, these traditions collectively inform contemporary yoga practices, characterized by meditation, breath control, self-discipline, and spiritual liberation. Such components of yoga can be traced to the Yoga Sutras of Patanjali, developed between 200 BCE and 400 CE, which formalized a series of principles that serve as the framework of Classical Yoga.

This practice hinges upon bodily and spiritual unity, in which physical health, mental clarity, ethical conduct, and spirituality are aligned. This idea of yoga, both traditionally and currently, is associated with preventative care, aiming to implement long-term wellness and maintain or restore one’s inner state of balance. Within this knowledge system, health and well-being are understood as a dynamic equilibrium centered on harmonious living.

Yoga can be practiced through a multitude of modalities, accommodating to a variety of lifestyle factors. Science has begun to recognize the widespread healing potential of yoga in aging populations. Although popularly regarded solely as a physical activity for strength, balance, and flexibility, yoga can serve as an equivalent mental exercise, simultaneously supporting cognitive function and mental acuity. As a popular and accessible traditional practice, yoga is beginning to bridge gaps in medicine by offering holistic and nonpharmacological alternatives to anti-aging interventions. This review highlights five studies that substantiate the potential for yoga to be a positive moderator in healthy cognitive aging in older adults. 

The first study evaluated the effectiveness of Hatha yoga—a slow, balanced, and breath-focused flow—in improving the cognitive health of 45 older adults, including attention, memory, and reasoning skills. Participants were divided among three trial groups: regular guided yoga sessions, other forms of physical activities, and a non-physically active control group. The yoga group experienced significantly greater improvements across all cognitive categories. Although the study was limited by its small sample size, the authors concluded that yoga-based healthcare interventions are an effective and holistic approach to maintaining and stimulating cognitive health in older adults. Their findings support yoga as a nonpharmalogical intervention that can improve the mental quality of life in elderly populations.

The second study examined the differential effects of Kundalini yoga (focused on spirituality and internal awareness) and memory enhancement training (MET) on hippocampal connectivity in older women with self-reported memory decline and cardiovascular risk factors for Alzheimer’s disease. The authors discovered that Kundalini yoga was associated with increased connectivity between a left anterior hippocampal subregion and ventral visual stream regions: areas of the brain linked to reduced perceived stress. It is hypothesized that Kundalini yoga’s combination of breathwork, movement, and mindfulness facilitates the processing of emotional content. Alternatively, MET increases connectivity between posterior hippocampal subregions and default mode/frontoparietal networks: regions that reduce frequency of forgetting information and experiences. This study provides compelling evidence that suggests that, together, Kundalini yoga and MET interventions may be beneficial for cognitive health and offer complementary benefits for stress management and memory acuity. 

The third article compared the effects of Hatha yoga and cued, high-speed yoga on the cognitive performance of 21 older adults. Cued yoga differs from traditional Hatha yoga by integrating visual and auditory cues into the practice. Participants completed several neuropsychological assessments before and after a trial period. They were randomly assigned to one of two groups: one group participated in three Hatha yoga sessions per week for 24 weeks, while the other group completed three cued yoga sessions per week for the same duration. The study reported improved cognitive test scores in both groups. However, participants in the cued yoga group showed greater improvements in retaining verbal information. This study concluded that both forms of yoga may be valuable tools for enhancing memory and reducing age-related cognitive decline. 

The fourth article reviewed how traditional yoga practice can offer holistic mind-body support for the WHO’s Decade of Health Aging criteria, including changing attitudes toward aging, creating age-friendly environments, delivering integrated and person-centred care, and ensuring access to long-term care. The authors used existing evidence to suggest that yoga improves memory, attention, executive function, stress regulation, and depressive symptoms while reducing risk factors associated with cognitive decline. The authors also called for expanding the evidence base for yoga healing and increasing accessibility for older adults through subsided community projects and tele-yoga programs.

The fifth study reviewed non-pharmacological mechanisms that offer neurobiological and anti-aging benefits, with a focus on holistic practices such as yoga and meditation. One section highlighted the mental and cognitive benefits of yoga, centered on stress reduction and enhanced memory, focus, mental agility, neuroplasticity and resilience as primary components that may produce anti-aging outcomes. The review underscored that yoga can support overall cognitive health and, therefore, potentially slow the aging process.

Enhancing Cognitive Health in Elderly Individuals: The Impact of Hatha Yoga on Attention, Memory, and Reasoning: A Randomized Controlled Trial

Abstract

Background

Aging leads to physiological and psychological changes that compromise both mental and physical autonomy, as well as cognitive functions, thereby increasing the risk of anxiety and depression. The sedentary lifestyle typical of older individuals results in a deterioration of the overall quality of life and well-being.

Objective

This study aims to evaluate the effectiveness of Hatha yoga in improving cognitive health among older adults. We will specifically examine the impact of this practice on attention, memory, and reasoning.

Methods

The present study assesses the impact of Hatha yoga on attention, memorization, and reasoning in healthy older adults aged between 65 and 80 years. The study population comprises 45 healthy individuals (26 men and 19 women; 72.3 ± 5.6 years) residing in a retirement home, divided into three groups: a yoga group (YOGA, n = 15) that participated in yoga sessions; a physical activity group (APS, n = 15) engaged in sports and physical activities sessions; and a control group (CONT, n = 15) that did not undertake any activities. The study spanned 24 sessions, with two sessions per week lasting 45 min each. Participants completed test sessions dedicated to evaluating attention, memory, and reasoning before (T0) and after (T1) 12 weeks. A two-way ANOVA was used to analyze the differences between groups and over time.

Results

After the intervention sessions, the data showed that the YOGA group registered significantly greater improvements at T1 compared to that of T0 in all cognitive parameters (e.g., attention (p < 0.001, Hedges’ g = 1.35), memory (p < 0.001, Hedges’ g = 1.04), and reasoning (p < 0.001, Hedges’ g = 1.82)). Furthermore, our results revealed a significant difference between the YOGA group and both the APS (p < 0.001) and CONT (p < 0.01) groups for the attention and reasoning parameters at T1.

Conclusions

This study underscores the potential of Hatha yoga to enhance the mental well-being of the elderly, suggesting significant benefits for cognitive well-being in this population.

Impact of Yoga Versus Memory Enhancement Training on Hippocampal Connectivity in Older Women at Risk for Alzheimer’s Disease

Abstract

This research investigates the differential effects of Kundalini yoga (KY) and memory enhancement training (MET) on resting-state hippocampal connectivity in older women with subjective memory decline and cardiovascular risk factors for Alzheimer’s disease (AD). Using a randomized controlled trial design, we examined changes in hippocampal subregion connectivity following 12 weeks of either KY or MET. Our results indicate that KY is associated with increased connectivity between a left anterior hippocampal subregion and ventral visual stream regions, correlating with reduced perceived stress. In contrast, MET showed increased connectivity between posterior hippocampal subregions and default mode/frontoparietal networks, which was associated with a lower frequency of forgetting. These findings suggest that KY and MET may engage distinct neural pathways, potentially offering complementary benefits for individuals at risk for AD.

Comparative Impacts Of Traditional Yoga And Cued, High-Speed Yoga On Older Adults’ Cognitive Performance: 1569

Abstract

Age is the greatest risk factor for cognitive decline. Hatha Yoga (HG) is a feasible practice for helping older adults to maintain or improve cognitive functions. YogaCue (YC) integrates visual and auditory cues into traditional Hatha practice to improve cognitive performance in cognitively healthy older adults.

PURPOSE

This study compared improvements in the cognitive performance of older adults following 24 weeks of HG or YC training.

METHODS

21 healthy older adults (YC = 11, HG = 10; 73.7 ± 4.6 y; Montreal Cognitive Assessment (MoCA) ≥ 24) participated in the study. Subjects completed a battery of neuropsychological measures, including the computerized National Institute of Health (NIH) Toolbox (Cognitive Module), the Hopkins Verbal Learning Test (HVLT), and the Trail Making Test parts A and B (TMT-A, TMT-B). Following pre-testing, subjects were stratified according to their score on the Montreal Cognitive Assessment (MOCA) and age, and randomly assigned to YC or the HG. Subjects attended three 1-hour sessions per week for 24 weeks.

RESULTS

Significant time effects were seen for the List Sorting Test (LST; p = 0.049, ηp2 = 0.189) and the Fluid Cognition Composite Score (FCS; p = 0.005, ηp2 = 0.352). Pairwise comparison revealed a significant increase in LST (Mdiff ± SE = 7.0 ± 3.4; p = .049; g = .61) and FCS (7.891 ± 2.46; p = 0.005; g = 0.59) following training. No significant main effects or interactions were seen for any other tests in the NIH Cognitive Toolbox. For the HVLT, there were significant time effects for Total Recall (HTR: p = 0.007, ηp2 = 0.328) and Delayed Recall (HDR: p = 0.027, ηp2 = 0.231) and a trend towards a significant time effect for Percent Retained (HPR: p = 0.051, ηp2 = 0.186). All changes were driven by improvements by YC (HTR 3.82 ± 1.32; p = 0.009; g = 0.77; HDR: 2.00 ± 0.69; p = 0.009; d = 0.49; HPR: 15.02 ± 5.96%; p = 0.021; d = 0.31). No significant main effects or interactions were seen for any other components of the HVLT or TMT-A or TMT-B.

CONCLUSION

Both YC and HG are valuable tools for improving memory and mitigating age-related cognitive decline. However, HVLT tests reveal that YC may have a greater impact on retaining verbal information than HG.

Yoga for Healthy Ageing: Evidence, Clinical Practice, and Policy Implications in the WHO Decade of Healthy Ageing

Abstract

Ageing is a dynamic biological process involving interconnected physiological, psychological, and social changes, making the promotion of healthy ageing a global public health priority. The World Health Organization (WHO) defines healthy ageing as the process of developing and maintaining functional ability that enables well-being in older age. The WHO’s Decade of Healthy Aging (2021–2030) outlines four key action areas: changing attitudes toward ageing, creating age-friendly environments, delivering integrated and person-centred care, and ensuring access to long-term care. This Perspective examines yoga, a holistic mind–body practice integrating physical postures, breath regulation, and mindfulness, as a potentially safe, adaptable, and scalable intervention for older adults. Evidence suggests that yoga may improve flexibility, balance, mobility, and cardiovascular function, reduce pain, and support the management of chronic conditions commonly associated with ageing. Psychological and cognitive research further indicates reductions in stress, anxiety, and depressive symptoms, alongside potential benefits for attention, memory, and executive function. Improvements in health-related quality of life (HRQoL) have been reported across physical, psychological, and social domains, with benefits sustained through regular practice. Adaptations such as chair-based practices, restorative postures, and the use of props enhance accessibility and safety, allowing participation across diverse functional levels. Mindfulness and breath-focused components of yoga may additionally support emotional regulation, resilience, and psychological well-being, particularly among older adults experiencing stress or limited mobility. Yoga interventions are generally well tolerated, demonstrate high adherence, and can be delivered through in-person and digital formats, addressing common access barriers. Despite this growing evidence base, yoga remains underintegrated within health policy and care systems in the US, UK, and India. Strengthening its role may require coordinated efforts across research, policy, and implementation to support healthy ageing outcomes.

Neurobiological and Anti-Aging Benefits of Yoga: A Comprehensive Review of Recent Advances in Non-Pharmacological Therapy

Abstract

Aging is an unavoidable process marked by numerous physiological and psychological changes that profoundly impact overall health and quality of life. As the global population ages, there is a growing interest in non-pharmacological therapies that can mitigate the adverse effects of aging and promote longevity. This narrative review explores the broad spectrum of non-pharmacological mechanisms offering anti-aging benefits, with a special emphasis on holistic practices such as yoga, meditation, and lifestyle interventions. These therapies influence critical biological processes, including reducing oxidative stress, modulating chronic inflammation, enhancing immune function, and preserving telomere length—key markers of cellular aging. Additionally, these practices contribute to improved cardiovascular health, musculoskeletal strength, and mental well-being, fostering a comprehensive approach to healthy aging. Yoga, for instance, combines physical postures, breath control, and meditation to enhance physical flexibility, mental clarity, and emotional balance. Meditation practices are shown to reduce stress and improve cognitive function, further supporting healthy aging. Lifestyle modifications, such as diet and regular physical activity, complement these therapies by promoting overall vitality and reducing the risk of age-related diseases. This review underscores the potential of integrating these non-pharmacological strategies into daily routines to enhance resilience against aging, improve quality of life, and extend healthspan. Furthermore, it highlights the need for continued research to fully elucidate the mechanisms by which these therapies exert their anti-aging effects and to develop evidence-based guidelines for their implementation.
